Registration is now completed entirely online. Beginning March 31, 2020 you do NOT need to sign and send a consent form. You may print a confirmation of your consent after you have registered and retain it for your own records.
The Alberta Organ and Tissue Donation Registry is the place to register your consent to become an organ and/or tissue donor.
It’s important that you understand what it means to be a donor and how important your choice is. Tell your family and loved ones that you have signed up to be a donor. They will then be prepared to speak on your behalf.

If you want to be an organ and/or tissue donor:
- talk about your wishes to be an organ and/or tissue donor with your family and tell them exactly what you want to happen when you die.
- access the online Organ and Tissue Donation Registry to record your choices about organ and/or tissue donation, or visit a Registry Agent office to register, or sign the back of your healthcare card
- call a Living Donor Program in your area if you want to be a living donor (kidney or part of the lung or liver, or tissue such as bone marrow or stem cells)

Becoming a Donor. The Organ and Tissue Donation Registry is a way to record your wishes to donate organs and/or tissues after you die. Follow the steps in the registry to give your consent about donating organs and/or tissues.
If you want to be a living donor (donate a kidney, part of your lung, or part of your liver while you are still alive), you need to contact the Living Donor Program in Calgary or Edmonton. For more information about consenting to donate, please see
Consent to Donate.People that need transplants are matched to an available organ based on many factors. Information is used to help medical specialists decide which recipient is the best match for the organ that is available.
Donor coordinators (nurses with special training) coordinate all organ donations and work with the eye and tissue programs in the province. They provide follow-up support and communicate with donor families.
